This is our Summer Bundle Meals Registration Form for the first week of summer break (6/21 – 6/25). We will be in seven CCSD schools providing weekly summer bundle meals from the week of June 21 through the week of August 2 (seven weeks). Any child age 18 or under can receive these weekly summer meal bundles at no cost. Pick-up days will vary per school but pick-up times will always be 11 am - 1 pm.
On this form, you can register up to four children (age 18 or younger) in your household. If there are more than four children in your household, please submit a second registration for those kids. The deadline to submit this registration form is 5 pm Thursday of each week. We will send out a new registration form link each week to sign up for the following week's meals. Meaning you will need to register weekly for your summer meal bundles.
When you pull up for your summer meal bundles, please have the following information for each child WRITTEN ON A SHEET OF PAPER (which you will give to us each time you pick up meals): child age, child first name, middle initial, last name, school name if applicable.
