The school district provides Medical Homebound Services if a student is unable to attend school as a result of an illness, accident or pregnancy. A medical doctor must determine if the student is unable to attend and must complete a Medical Homebound Instruction Form. A certified teacher will provide instruction in the student's home or the public library. The student’s teachers will provide assignments and tests during the homebound period.
It is very important that the Medical Homebound Application Form be completed in a timely manner to ensure that services are provided as soon as possible once it is determined the student is not able to attend school.
