Health Emergency Card
Parents or guardians must complete the Health Emergency Card at the start of every school year or if the student transfers to another CCSD school. The Health Emergency Card is available in Spanish or bilingual formats. Please update information on the student emergency card if you move, change contact telephone numbers or if the student’s health status changes.
