Nursing Services Field Trips
Parent Responsibilities
- Notify teacher and nurse if your child has a special health care need. The health care provider must complete the CCSD Medication/Procedure Doctor’s Order form in order for the student to have medication at school and on field trips.
- Over the Counter Medications (OTC) will not be provided on field trips without a completed CCSD Medication/Procedure Doctor’s Order form.
- Medications will be the responsibility of the trained designated CCSD employee.
- Contact your school nurse with any medication questions, and/or about your child carrying his/her own medications or see above paperwork to be completed for self medicating or monitoring.
Teacher Responsibilities
- Inform the school nurse of field trip dates, students and chaperones attending at least one week in advance. Minimum two week notification to school nurse for overnight field trips. More time would be appreciated.
- Follow district policy regarding assisting students with routine medications while on the field trip.
- The school nurse will provide training for emergency medications/procedures if you have students who require them. (You may be asked to complete a Safe Schools module prior to your individualized training.)
- Licensed CCSD professionals (RN/LPN) are the only employees allowed to administer/supervise insulin administration.
- Sign out for medications with your school nurse (or designated CCSD employee) on the field trip medication sign out sheet.

- Plans may need to change if the teacher is out day of field trip. Contact your school nurse with any questions or concerns.
School Nurse Responsibilities
- Remind your principal and teachers that you need to be informed about dates/times of field trips with list of students and chaperones attending, at least one week in advance.
- Review names of field trip attendees in order to determine training needs.
- Provide appropriate trainings for emergency medications/procedures based on emergency action plans.
- Request that teachers complete Safe Schools online training as appropriate. School nurse should keep documentation of training in clinic.
- Activate the field trip plan for routine medications
